Attitudes Survey
Reader Attitude survey
This survey is an informal assessment used to determine student attitudes towards reading and student attitudes toward their own ability to read. The target ages are five to eight and the time required is ten to fifteen minutes. Depending on student abilities, the teacher can read the question aloud or have the students read them individually. The students will need a pencil to circle the picture that best describes their feelings towards reading or crayons to color the picture that best describes their feelings towards reading.

Who am i as a reader?
This survey is an informal assessment used to determine student attitudes towards reading. The target ages are eight to twelve and the time required is ten to fifteen minutes. The students are to work individually to read each question. On the first part of the survey, the students will need a pencil to circle the answer that best describes their feelings. On the second half of the survey, the students will write the answer to the questions.
